PharmD Application Resources

Thank you for your interest in becoming a student at Idaho State University! The first step in this process is to fill out a bit of paperwork. Below are a series of links that will get you started. We strongly recommend that you read the Letter document so that the process is clear. Personal Recommendation A form to be filled out by professional, work or academic supervisors
Pharmacist Recommendation A form to be filled out by licensed pharmacists
